This policy explains how Crystal Ladhar D.D.S. handles information collected through crystalladhardds.com. It covers this website only. The way we handle your medical and dental records inside the practice is governed separately by our Notice of Privacy Practices under HIPAA, which we provide at your first visit and can send you on request.

Information you give us

When you request an appointment, ask us to verify your insurance, or contact us through this site, we collect what you type into the form. That typically means your name, phone number, email address, insurance details, and whatever you tell us about why you want to be seen. Some of that may be health information, and we treat it accordingly.

Please do not use website forms to send us anything urgent. If you are in pain or think you have an infection, call the office at (408) 252-5575. If you have a medical emergency, call 911.

Information collected automatically

Like most websites, this one records basic technical information when you visit: your IP address, browser and device type, the pages you looked at, and the site or search that sent you here. We use cookies and similar technologies to keep the site working and to understand which pages people actually read.
