These terms cover your use of this website. They are not a treatment agreement, and they do not change anything we tell you in the office. If you do not agree with them, please do not use the site.
Using this site
You are welcome to read this site, request an appointment, send us your insurance details, and contact the office through it. Please do not attempt to break into it, scrape it, overload it, or use it to send anything unlawful. We may change or remove any part of the site at any time, and we may stop offering a feature without notice.
The content here is information, not dental advice
Everything on this site, including treatment pages, blog posts, and answers to common questions, is general information. It is not a diagnosis, not a treatment plan, and not a substitute for being examined by a dentist. What is right for someone else may be wrong for you, and we cannot tell which without seeing you.
Reading this site does not make you our patient, and neither does sending a form. That relationship starts when we see you in the office.
Please do not use this site for anything urgent. If you are in pain or think you have an infection, call the office at (408) 252-5575. If you have a medical emergency, call 911.
Appointment requests
A request sent through this site is a request, not a booked appointment. It becomes an appointment when we contact you to confirm it.
What belongs to us
The text, photographs, layout, and marks on this site belong to Crystal Ladhar D.D.S. or to the people we licensed them from. You are welcome to read and print pages for your own personal use. Please do not republish, sell, or use them commercially without our written permission, and please do not use our name or marks in a way that suggests we endorse something we do not.
Links to other sites
This site links to third parties, including our scheduling tool, third-party financing providers, and our social media pages. Once you follow one of those links you are on their site, under their terms and their privacy policy, not ours. We link to them because they are useful, not because we control what they do.
Privacy
How we handle information collected through this site is covered by our Privacy Policy. The way we handle your dental records inside the practice is governed separately by our Notice of Privacy Practices under HIPAA, which we provide at your first visit and can send you on request.
Limits on our responsibility
We put real effort into keeping this site accurate and available, but we cannot promise it is free of errors or that it never goes down, and we would rather say that plainly than imply otherwise. To the extent the law allows, we are not liable for losses arising from your use of this site or from relying on its general information instead of being examined. Nothing here limits responsibility that the law does not allow us to limit, and nothing here changes the standard of care we owe you as a patient.

We may update these terms. Changes will be posted on this page rather than made quietly, and continuing to use the site after they are posted means you accept them.
