Many adults who ask about straightening their teeth have thought about it for years. Some had braces as a teenager and stopped wearing their retainer. Some never had treatment at all. Others have watched their lower front teeth slowly crowd together and are not sure whether it is worth fixing now.

For most of them, clear aligners are a good place to start the conversation. They are nearly invisible, they come out to eat and brush, and they let you straighten your teeth without anyone at work noticing.

Aligners also ask more of you than braces do. They only work while they are in your mouth, and the result only lasts if you wear a retainer afterward. Knowing that up front makes it much easier to decide whether they fit your life.

This guide covers why adult teeth shift, how aligners work, who is a good fit, what treatment is like, how long it takes, why retainers matter, the questions worth asking, and what cost and insurance usually look like. Crystal Ladhar D.D.S. is a single-doctor practice in west San Jose, and aligner treatment here is planned and followed by Dr. Crystal Ladhar.

Why Adult Teeth Shift

Teeth are not fixed in place. They sit in bone that keeps changing for life, and they slowly drift forward and inward over the years. That is why lower front teeth that were straight at 20 often overlap by 40.

If you had braces as a teenager, the most common cause of change is simple. The retainer stopped being worn, and the teeth moved back toward where they started. That is not a failure of the original treatment. It is what teeth do without a retainer.

Other causes include gum changes, a missing tooth that lets its neighbors tip into the gap, and grinding at night. Each one affects the plan, which is why treatment starts with a full exam rather than a quick look at the front teeth.

How Clear Aligners Work

Clear aligners are a series of thin, clear, removable trays. Each set is shaped slightly differently from the last. You wear each set for one to two weeks, then switch to the next, and your teeth move a little with each step.

Treatment starts with a scan from the iTero scanner instead of impression putty. The whole series of moves is planned digitally from that scan, so you can see where your teeth are headed before you begin.

The 20-hour rule

Aligners only work while they are in. Plan to wear them 20 to 22 hours a day, taking them out only to eat, to drink anything other than water, and to brush. If that is not realistic for your life, say so at the consultation and we will talk about what else might fit.

Who Is a Good Candidate for Clear Aligners

Aligners work well for adults and teens with:

  • Crowded or overlapping teeth
  • Gaps between teeth
  • Teeth that have shifted since braces years ago
  • Mild to moderate bite problems
  • Crowding that makes teeth hard to clean

A few things need to be in place first. Your gums should be healthy, and any cavities or gum disease should be treated before teeth start moving. Crowns, bridges, and implants also affect the plan, since implants do not move at all. Some complex bite problems are better handled by an orthodontist, and if your case is one of them, you will hear that plainly.

What Treatment Is Like Day to Day

The first few days

Each new set of trays brings mild pressure for a day or two, then it fades. Speaking can feel different for about a week. Most people stop noticing the trays soon after.

Eating and cleaning

The trays come out for every meal, so there is no list of foods to avoid the way there is with braces. Brush before you put them back in, so food is not trapped against your teeth. Straighter teeth are also easier to keep clean once treatment is done.

Check-in visits

You come in from time to time so Dr. Ladhar can confirm your teeth are moving with the plan. Small adjustments along the way are a normal part of treatment.

How Long Clear Aligner Treatment Takes

Simple cases, such as mild crowding or a small gap, often take about six months. More involved cases run twelve to eighteen months. Dr. Ladhar gives you a range after your scan rather than a guess before it.

The biggest factor you control is wear time. Trays left out for hours at a time slow everything down and can add months to treatment.

Retainers: The Part That Keeps Your Result

When the aligners finish, the work is not quite done. The fibers and bone around your teeth keep remodeling for life, and without a retainer, teeth drift back. That is the main reason so many adults end up in aligners a second time.

A custom retainer is made from a scan taken as soon as treatment finishes, while your teeth are exactly where you want them. It can be removable and worn at night, or a thin wire bonded behind the front teeth. Most people wear it every night at first, then settle into a few nights a week for the long term.

Plan to replace a removable retainer every few years, since they crack, warp, and get lost. If one feels tight after a few weeks out, call us rather than forcing it in.

Ten Questions to Ask Before Starting Clear Aligners

Ask these at any practice you are considering. The answers show how the office thinks as much as what it recommends.

  1. Is my case a good fit for aligners? You want a clear answer, with the reasons behind it.
  2. Can I see where my teeth are headed before I commit? A digital plan from your scan shows what treatment is aiming for.
  3. Are my gums and teeth healthy enough to start? Cavities and gum disease should be treated first.
  4. How will my crowns, bridges, or implants affect the plan? Dental work changes what can move.
  5. What is my estimated timeline, in writing? A range helps you plan around work and events.
  6. How often will I need to come in? Busy adults should know this up front.
  7. What happens if my teeth do not move as planned? Ask how extra trays and adjustments are handled.
  8. What retainer will I need, and is it included? Retention protects the result for life.
  9. What could change the fee during treatment? Missed wear time, lost trays, or extra adjustments can add cost.
  10. What if aligners turn out not to fit my life? Ask what other options you would have.

What Clear Aligners Cost

Fees depend on how complex your case is and how long treatment takes, so any number quoted before a scan is a guess. At our office, Anna, our treatment coordinator, prices your full plan in writing before you agree to any of it.

Ask what the quote includes, such as your scan and planning, your full series of trays, check-in visits, and your first retainer. Also ask what could add to it, such as lost trays, extra trays for adjustments, or replacement retainers later on.

To spread out the cost, established patients can arrange in-house payments with Anna, third-party financing is available to any patient at no extra cost, and patients 65 and over take five percent off treatment. Many adults also use funds from a flexible spending account (FSA) or health savings account (HSA). Check with your plan administrator to confirm.

Does Insurance Cover Clear Aligners for Adults?

Adult orthodontic coverage is less common than coverage for children. Many dental plans limit the orthodontic benefit to dependents under a certain age. Some employer plans include an adult benefit, usually a percentage of the fee up to a lifetime maximum, so it is worth checking.

We are out of network with every insurer and accept all PPO plans. Linda, our billing coordinator, files your claim and follows up when it stalls. Before you start, send your plan details through our benefits check and Linda will tell you what to expect. We do not accept HMO plans or medical insurance.

Red Flags When Comparing Aligner Options

Most dental offices in San Jose do good work. These patterns deserve a second look:

  • Aligners mailed to you without an in-person exam or X-rays
  • A treatment plan made without checking gum health
  • No discussion of how crowns, bridges, or implants affect the plan
  • Aligners recommended for every case, regardless of the bite
  • No plan, or no price, for retainers after treatment
  • A low price with no written breakdown of what is included

How Our Office Approaches Clear Aligners

Every aligner case here starts with a full exam, X-rays, and an iTero scan. Your gums are checked first, because healthy gums are what make tooth movement safe. If aligners are not the right fit for your bite, you will hear that plainly, along with what would fit better.

The plan is priced in writing before you start, and your retainer is part of the conversation from the first visit, not an afterthought at the end.

None of this replaces an in-person exam. Timelines, options, and fees are set only after your teeth and bite have been examined.
